Layered Process Audit Template

A layered process audit (LPA) confirms that the critical few controls — the ones that protect quality and safety — are actually being followed, every shift, at every level of the organization. Operators check daily, supervisors weekly, managers monthly. The template keeps each layer short, focused, and consistent.

When to use it

The situations it fits

  • When standards exist on paper but drift between shifts or lines
  • When defects escape even though 'we have a procedure for that'
  • When quality depends on inspection catching problems instead of process control preventing them
  • When you want leaders at every level verifying the same critical controls

How to use it

Step by step

  1. 1.Identify the critical few controls — the steps where a miss creates real quality or safety risk
  2. 2.Build short checklists for each layer: daily for operators, weekly for supervisors, monthly for managers
  3. 3.Audit the process as it actually runs, not as the procedure says it should
  4. 4.Log every miss as an action with an owner and due date
  5. 5.Review trends monthly — repeated misses mean the standard or the process needs to change

Illustrative example

What it looks like in practice

A food plant lists five critical controls for its filling line: seal temperature, torque check, allergen label verification, metal detector challenge, and sanitation sign-off. Operators confirm all five each shift, the supervisor audits two lines weekly, and the plant manager audits one area monthly. A repeated miss on torque checks traces back to a worn calibration gauge — fixed once, instead of re-checked forever.

This example is illustrative, not a client case study.

Common mistakes

What to avoid

  • Auditing everything instead of the critical few — long checklists get pencil-whipped
  • Using audits to blame people instead of finding why the standard is hard to follow
  • Skipping the management layer, which signals the checks don't really matter
  • Logging misses without turning them into actions and root-cause fixes
